ABSTRACT

Objective To explore the clinical efficacy of intensity-modulated radiotherapy combined with chemotherapy in the treatment of advanced cervical cancer. Methods All 121 patients with advanced cervical cancer (stage Ⅱ B &Ⅲ A & Ⅲ B) selected in our hospital from June 2012 to June 2014, who were treated with combined chemoradiotherapy, were divided into observation group [intensity modulated radiation therapy (IMRT)group)] and control group [3-dimensional conformal radiation therapy (3D-CRT) group].There was no significant difference between two groups in mean age, body mass index, International Federation of Gynecology and Obstetrics (FIGO) clinical stage, pathological type and chemotherapy mode (P > 0.05).The clinical features of two groups were compared. The treatment and follow-up of two groups were recorded. Results The bone marrow suppression [32.2% (19/59) vs 51.6% (32/ 62)], gastrointestinal reaction [42.4% (25/59) vs 62.9% (39/62)], and rectal reaction rate [27.1% (16/59) vs 45.2% (28/62)] of the observation group were significantly less than that of the control group (P < 0.05).The incidences of genitourinary tract injury [18.6% (11/59) vs 24.2% (15/62)], radiation proctitis [23.7% (i4/59) vs 25.8% (16/62)] and radiation cystitis [16.9% (10/59) vs 20.9% (13/ 62)] of two groups had no significant difference (P > 0.05).Two groups were followed up for 3 years, the local control rate of the observation group was significantly higher than that of the control group (86.3% vs 70.1%) (P < 0.05).Conclusions IMRT combined with chemotherapy in the treatment of advanced cer vical cancer can improve the local control rate of tumor, protect the endangered organ, and reduce the side effects of radiotherapy.

ABSTRACT

Objective To explore the effect of miR-200b on chemosensitivity of cervical cancer HeLa cells to paclitaxel and its mechanism. Methods A recombinant miR-200b mimics was transfected into cultured HeLa cells, which were divided into observation group, negative group, and blank control group. Each group was treated with different concentrations of paclitaxel.Methyl thiazolyl tetrazolium (MTF) assay was performed to evalate the proliferative ability of these treated cells to paclitaxel.AnnexinVFITC/PI method was used to detect the apoptosis rate of Hela cells. The protein level of vascular endothelial growth factor (VEGF) was detected by Western blot. Results MTT assay showed that the OD values of three groups were totally different;and compared to the other two groups, the OD value of the transfected HeLa cells was significantly lower (P < 0.05).Compared to the negative group and the blank control group, there was no statistical difference (P > 0.05).AnnexinV-FITC/PI results demonstrated that the apoptotic index of the transfected HeLa cells was significantly higher than that in the other two groups (P <0.05).Western blot assay showed that expression of VEGF protein in three groups of relative quantity was 0.403 ± 0.046,0.882 ± 0.094,and 0.901 0.102,respectively.There was significant difference among there groups (P < 0.05).Conclusions miR-200b mimics can increase the sensitivity of cervical cancer HeLa cells to paclitaxel, and targeted regulation of VEGF may be the cause of increasing in drug sensitivity.

ABSTRACT

Objective To investigate the effect of neoadjuvant chemotherapy on the prognosis of patients with cervical cancer. Methods A systematic review of the literature about neoadjuvant chemotherapy and radical surgery was performed with searching Pubmed,Cochrane library,Medline, and Embase database from January 1990 to January 2015.According to the inclusion and exclusion criteria, the data were extracted and analyzed by Revman 5.3 system software Meta. Results Ten controlled clinical studies were concluded, including 1 543 patients (667 in preoperative neoadjuvant chemotherapy group and 876 in radical surgery). The extracted data were comparable.Meta-analysis results displayed that significant difference existed in five-year disease-free survival,node-positive rate, vascular invasion rate, and parametrial infiltration rate.It seemed that the preoperative neoadjuvant chemotherapy was prior to the radical surgery in those items. There was no significant difference between the preoperative neoadjuvant chemotherapy and radical surgery in five year survival, intraoperative complication, the positive surgical margin rate, and five-year survival rates among different chemotherapy peers (P > 0.05).Conclusions For locally advanced cervical cancer patients, neoadjuvant chemotherapy can reduce the incidence of high-risk pathology, but had no significant effect on the long-term survival of patients.

ABSTRACT

Objective To explore the efficacy and safety of irinotecan as neoadjuvant chemotherapy (INAC) plus radical surgery (RS) for cervical cancer. Methods According to International Federation of Gynecology and Obstetrics (FIGO),81 cases were divided into Ⅱ B,ⅡA,and Ⅰ B2 groups.According to the tests of UGT1A1 gene polymorphisms,we adjusted the injection dose of irinotecan. The parameters were analyzed, including the efficacy, operation time and bleeding volume, postoperative pathology, survival time, and adverse reactions. The articles on irinotecan or paclitaxel combined with cisplatin for neoadjuvant chemotherapy between 2005 and 2015 were collected, and compared. Results The effective rate of chemotherapy was 81.5% (Ⅰ B2 group:85.7%;Ⅱ A group:83.3%;and ⅡB group:72.2%),operation time was (5.3 ± 1.1) h,and blood loss was (781 ± 361.7) ml.After chemotherapy,37 cases were delayed diarrhea,70 cases were nausea,48 cases were vomiting, and 40 cases were bone marrow suppression. The infiltration rate, operation time, and blood loss on Ⅱ B group was significantly higher than that on Ⅱ A and Ⅰ B2 groups(P < 0.05),and there was no significant difference in the chemotherapy efficiency, invasion depth, lymphatic metastasis, survival time and adverse reactions(P >0.05).Compared to three articles, the total effective rate in this study was higher than that in previous studies, also in Ⅱ A and Ⅱ B group. Conclusions Irinotecan chemotherapy regimens combined with cisplatin is effective and well tolerated. It is worthy of popularization and application.Detection of UGT1A1 gene polymorphism has guiding significance for chemotherapy regimen on irinotecan combined with cisplatin.

ABSTRACT

Objective To explore the effect of black garlic extract on HeLa cells and its possible molecular mechanisms. Methods Inhibitory rate of HeLa cells was detected with methyl thiazolyl tetrazolium(MTT).Flow cytometry annexin V-fluorescein isothiocyanate/propidium iodide (V-FITC/PI) was used to detect tumor cell apoptosis.Flow cytometry was used to measure cell-cycle changes.Immunohistochemistry method was used to detect expressions of tumor proteins bax and Bcl-2.Results Black garlic extract significantly inhibited the growth of HeLa cells.Black garlic extract significantly induced apoptosis of HeLa cells. The apoptosis rate was (53.26 ± 1.78)% in the black garlic high-dose group, and (3.68 ±0.11)% in the control group.Black garlic extract affected cell cycle, upregulated bax expression, and downregulated bcl-2 expression. Conclusions Black garlic extract significantly induced the apoptosis of HeLa cells. This effect might be caused through increasing G2/M cells or changing expressions of bax and bcl-2.
